Notice of privacy practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Last updated: October 24, 2026 (effective date)
Our commitment
At Healthy Solutions for Children and Families (HSCF), we protect the privacy of your health information. The federal HIPAA law requires us to protect your identifiable health information (called "protected health information"), to give you this notice of our legal duties and privacy practices, and to follow the notice currently in effect.
When a Florida law protects your information more than HIPAA does, for example the confidentiality between therapist and client, we follow the law that gives you more protection.
How we may use and disclose your information without your authorization
We may use and share your information for:
- Treatment: to provide your care and coordinate it with other professionals who treat you, for example your doctor or a psychiatrist.
- Payment: to bill and collect for services from you or your insurance, for example to verify your coverage or request an authorization.
- Health care operations: to run the practice, improve the quality of care, train staff and complete audits.
- Reminders and communications: to remind you of appointments or tell you about treatment options.
We may also share it with vendors who help us (for example, billing or electronic record systems) only if they sign an agreement that requires them to protect it.
Other uses and disclosures the law allows or requires
- When the law requires it.
- To report suspected abuse, abandonment or neglect of a child, an elderly person or a vulnerable adult, as Florida law requires.
- To prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
- For public health and health oversight activities, such as inspections or professional licensing.
- In court or administrative proceedings, when there is a court order or another valid legal process.
- To law enforcement, in the limited situations the law allows.
- To coroners or medical examiners, and for organ donation.
- For approved research that protects your privacy.
- For special government functions, such as national security, and for workers' compensation as the law allows.
- To a family member or person involved in your care, if you don't object or if, in an emergency, we believe it is in your best interest.
Uses that require your written authorization
- Psychotherapy notes: a therapist's personal notes, kept separate from your record, are shared only with your authorization, except for the limited exceptions the law allows.
- Marketing: we do not use your health information for marketing without your authorization.
- Sale: we never sell your health information.
Any other use not described in this notice requires your written authorization. You may revoke it in writing at any time, except for what we have already done based on it.
Your rights
- See and get a copy of your record, on paper or electronically. We may charge a reasonable, cost-based fee.
- Ask for a correction if you believe your information is wrong or incomplete. If we say no, we'll tell you why in writing.
- Ask for confidential communications: for example, that we call you at a different number or write to a different address.
- Ask us to limit how we use or share your information. We don't always have to agree, but if you pay for a service fully out of pocket and ask us not to share it with your insurer, we must agree.
- Get a list of certain disclosures we made of your information in the last six years.
- Get a paper copy of this notice, even if you agreed to receive it electronically.
- Choose a representative: if someone has your medical power of attorney or is your legal guardian, they can exercise your rights.
- Be notified if a breach affects your information.
To use any of these rights, write to our Privacy Officer (details below).
Minors
Parents or guardians generally exercise a minor's rights. Florida law lets some minors receive certain mental health services on their own; in those cases we protect their information as that law requires.
Our duties
- Keep your protected health information private and secure.
- Tell you promptly if a breach occurs that may have compromised your information.
- Follow the practices in this notice and give you a copy of it.
- Not use or share your information other than as described here, unless you authorize us in writing.
Changes to this notice
We may change this notice, and the changes will apply to all the information we have about you. The current notice will be on this page and in our office, and we'll give you a copy if you ask.
Complaints
If you believe we have not protected your privacy, you can file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights: 200 Independence Avenue, S.W., Washington, D.C. 20201, phone 1-877-696-6775, or at www.hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
